Use this COVID-19 vaccine checklist (based on FDA Fact Sheets of Authorized Vaccines) to do the following: 1. Assess whether you should receive the vaccine based on previous allergic reactions to COVID-19 vaccine ingredients or to a previous dose of the vaccine 2.
