Permit to Dig
To be filled out be a Foreman / Supervisor for every site before you dig greater than 300mm below original ground level. Permit to dig is to be completed BEFORE work begins. NOTE: For all trenches over 1.5m deep a Trench / Excavation Inspection Form must also be completed. If the answer to any item on this checklist in 'No', then work cannot proceed until the relevant item has been completed, or the risk assessed and the hazard mitigated. Note details of the mitigation once resolved. EXCEPTION A Permit to Dig is not required for hand digging or hydro-excavation, provided the task is assessed as Low Risk, and insulated hand tolls are used.
