This 3-month probation review template helps facilities management teams assess new starters at the 90-day mark. Capture employee details, overall performance, attendance and timekeeping, appearance, teamwork and communication, and client or customer feedback. Verify key competencies including understanding of line management and rota, scope of services, staff and health and safety handbooks, equipment and uniform readiness, training needs, and system testing such as AOVs, emergency lights, fire and lift alarms. Confirm reporting procedures for issues, sickness, lone working, and time off. Summarize performance, list actions, and record signatures from the employee, line manager, and head supervisor to finalize the review.
