This damaged infrastructure report template helps telecom teams capture and investigate fiber network incidents in a consistent way. Record incident identifiers, date and time, affected suburbs and clusters, MTTR and diagnostic time, responsible managers and reporters. Document damage details including reason for detection, number of services and ISPs affected, and NOC reporting with Active Teams references. Pin the location, list street details and GPS coordinates, and attach as built diagrams. Specify cable type, size and labels, describe the extent of damage, and add images and video. Capture witness statements, SAPS case details and investigator information when applicable.
