Use this Fitness Evaluation template to capture client details and assess baseline fitness across key areas. Document cardiovascular performance with a 1-mile walk test, measure strength with pushups, and optionally record flexibility and other body metrics. Include the location, evaluator, date, and signatures for staff and participant to verify results. Add notes and recommendations to guide training goals and track progress over time.
