Use this Confidentiality Sign-Off form to formalize privacy obligations for employees working in a medical practice. The template records the employee and manager names and signatures and confirms understanding that patient and staff information must not be disclosed, that misuse of internet or email is prohibited, and that confidentiality obligations continue indefinitely. It highlights potential disciplinary and legal consequences for breaches and supports compliance with data protection laws. Ideal to include with new starter onboarding (Part B) when roles require agreement to the practice’s confidentiality policy.
