Capture site, date, time, location, and people involved (personnel, patient, public, contractors, outside agencies). Record environmental conditions (inside/outside, lighting, wet/dry, level/slope) and classify the event, including near miss, injury types, and hazard categories such as slip/trip, ergonomics/handling, fire, electrical, working at height, construction, driving, equipment/mechanical, lone working, confined space, water, environmental factors, and animal/insect.
