Use this comprehensive incident report template to document dangerous occurrences, incidents, and near misses. Capture site, date, time, location, and people involved (personnel, patient, public, contractors, outside agencies). Record environmental conditions (inside/outside, lighting, wet/dry, level/slope) and classify the event, including near miss, injury types, and hazard categories such as slip/trip, ergonomics/handling, fire, electrical, working at height, construction, driving, equipment/mechanical, lone working, confined space, water, environmental factors, and animal/insect. Assess outcomes and note if the incident is RIDDOR reportable, including HSE Form 2508 types. Document injuries, affected body parts, full event details, and remedial actions to make safe and prevent recurrence. Suitable for healthcare and general workplace safety reporting.
