FTTH Splicing Audit Checklist
This FTTH splicing audit checklist helps telecom field teams document and verify fiber optic work quality. Capture site information, job references, and walk-off dates. Record SN and ASN details with photos of closed and open cabinets. Include images of splice trays before and after labeling, hydra cables or pigtails, and the splicing schedule. Track which fibers were spliced, whether a splitter was installed, and provide splitter photos. Document DP location with internal photos, confirm pre-con blown fiber installation, and attach splicing photos. Verify POE installations with internal images, add notes, and complete sign-off to standardize quality and compliance across FTTH installs.

Use this 4th Utility Walk Off Report CGI V1 to document fiber installation completion and quality. The checklist covers headend cabinet setup and labeling, splitter panel and fiber tray, power and EAD status, and FDP installation, labeling, and splicing. It also includes housekeeping items such as firestopping, cable routes, containment, non combustible fixings, hallway clips with white mastic, and POE placement and splicing. BRSK Fiber Site Walk Off Report
This site walk off report template is designed for BRSK fiber builds to document completion quality before handover. Inspect splitter boxes for correct location, labeling, and neat splicing; record units spliced and any connected FDPs. Verify FDP installation and labeling against the job pack. Check cable routes for compliance, use of non combustible fixings, containment, and correct hallway clips and mastic when using Invisilight or internal cable. Review PoE locations, splicing quality, and test levels, and ensure results are sent to BRSK. Confirm appropriate fire stopping materials are used and the fire stopping report is submitted.
