Use this fiber optic splicing report template for field crews working on NOLA One Fiber projects. Log customer, work order, start time, site location, crew roles, and work completed such as splicing, case re-entry, panel installation, troubleshooting, and testing. Specify splice types including butt, ring tap, and fiber turns; record equipment and closures like CommScope 24F and 72S; detail MST/FDT types and tray counts. Track cable A–D identifiers, directions, enclosure and pipe footage markers, and PID numbers. Attach photos of panels, trays, slack baskets, closures, tags, and restoration. Record completion and NOC release times. Complete the pre-task checklist covering weather, PPE, traffic control needs, equipment inspection, site safety orientation, hazards, and additional comments.
