Sub-contractor Customer Damage Complaint Checklist

Use this inspection template to capture and resolve customer damage complaints linked to subcontractor works. Record complainant details, site location, and work order. Conduct a WHS risk assessment for hazards such as working at height, ladders, EWP, weather, and hazardous materials, and document controls and residual risk. Describe the damage, severity, location, and likely causes. Attach mandatory photos and evidence. Confirm if damage relates to installation, outline actions, and track customer satisfaction. Document agreed resolutions, deed of release, insurance claim status, and email confirmation. Obtain contractor and complainant signatures to complete the report.
