This geotechnical and mining observation sheet helps teams document ground conditions and support in mines or similar excavations. Capture the number and orientation of joint sets, joint spacing, persistence, and roughness. Record distance of rock bolts from the face, drive axis orientation relative to major foliation or joint sets, drive size or section, and the standard bolting pattern with any gaps. Note rock bolt length, grouting, bearing plates and tensioning, and assess scaling quality. Identify loose rock and size, cable bolting, presence and quality of wire mesh, socket plugs, and any loose bars or bottom holes. Verify compliance with specified support requirements, grouting quality, and document any modifications to the support pattern with detailed remarks and conclusions.
