Use this Power Outage Report Form to document electrical interruptions affecting manufacturing operations. Capture date, time of outage and restoration, total downtime, immediate actions taken, and whether vulcaniser cycles were restarted. Record reasons for delays, number and type of sleeves rejected, affected vulcaniser numbers, and relevant part numbers. Engineering can log steps to restore power, verify boiler status, confirm vulcaniser pressure at 8 bar, and define corrective actions to prevent recurrence. Include additional comments to support root cause analysis and continuous improvement.
