Capture causes including tree or branch, vehicle accident, squirrel or bird, bad secondary, URD outage, consumer equipment, unknown, fire, bad transformer, loose connection, or other. Note right of way details, conductor guard added, and component affected at meter base, transformer, weather head, or secondary splice. Enter approximate outage location, comments, and add photos. Track KVA changes with old and new values and engineering clearance. Summarize number of consumers affected, outage duration, major outage status, notes, locate ticket number, and signature for completion.
