This BLEC Trouble Report template is designed for electric utility teams to document outages and service interruptions. Record who received the call, caller details, address, pole location, and the type of trouble such as outage, partial power, flickering lights, tree in lines, broken lines or pole, or fire. Log assigned crews, personnel involved, and time on site. Capture causes including tree or branch, vehicle accident, squirrel or bird, bad secondary, URD outage, consumer equipment, unknown, fire, bad transformer, loose connection, or other. Note right of way details, conductor guard added, and component affected at meter base, transformer, weather head, or secondary splice. Enter approximate outage location, comments, and add photos. Track KVA changes with old and new values and engineering clearance. Summarize number of consumers affected, outage duration, major outage status, notes, locate ticket number, and signature for completion.
