Medical Questionnaire for New Food Handlers

This new employee medical questionnaire template supports compliance with employment health, safety, and food safety legislation. It captures personal details and screens for conditions relevant to food handling, including enteric fever, recent diarrhoea or vomiting, skin conditions of the hands, arms, or face, ear/eye/nose/throat infections, food allergies, asthma, recurrent chest problems, seizures or blackouts, and colour blindness. It also asks about recent travel, includes space to provide details for any yes responses, and requires an employee declaration. A management sign-off confirms the individual is fit to work as a food handler. Information is kept confidential in line with data protection requirements.
