Hoyer Lift Observation Checklist

Evaluate safe patient transfers using a Hoyer or total body lift. This checklist guides staff through sling selection and inspection, communicating with the person served, adjusting bed height, securing wheelchairs, positioning the lift and spreader bar, attaching loops, raising and lowering safely, and removing the sling. Record pass or fail results and add recommendations for training or corrective action.

Use this Lifting Operations Compliance Checklist (LOCC) to standardize pre-lift checks and documentation. Verify a lift plan and expiry, establish a safe working area, and confirm the appointed person and lift supervisor per BS 7121. Record operator licenses, slinger and banksman presence, and daily checks. Capture HIAB certification, sling and chain serial numbers and certificates, and ensure equipment is in date. Confirm method statement and risk assessment are signed, assess ground conditions and spreader mats or plates, add photos as evidence, and complete sign-off for compliance.
