STEMC Field Observation Checklist

This STEMC Field Observation checklist supports utility field crews in assessing safety and compliance on site. It covers job planning and documented briefings, proper use of PPE (eye, head, hand, FR clothing, footwear, special PPE), and readiness of vehicles and equipment including documentation, wheel chocks, extinguishers, AEDs, first aid and BBP kits, lighting, trailers, grounding, and load security. It also reviews traffic control (signs, cones, flagging), energized work practices and minimum approach distances, general construction and maintenance checks, and climbing and elevated work requirements. Space is provided to note other safety concerns and capture signatures.

Crew Observation Form for Utility Safety
Use this crew observation form to audit utility field crews and document safety and work practices. Record job type, date, preparer, location and list crew members. Verify compliance around energized lines, minimum approach distances, vehicle grounding, PPE in use, hotline tags and cover-up, a qualified observer, lockout/tagout, proper grounds, and completed job briefings and hazard reviews. Assess work zones and traffic control, trenching and excavation, chocks and outrigger pads, cones and lighting, equipment clearances, and load security.
