Use this KEC Work Site Safety Observation Report to document field safety checks for electric utility crews. Record location, date, observer, crew members, job type including overhead, underground, substation, transmission, and scope. Verify vehicles and traffic control, wheel chocks, outriggers, grounding, DOT walk around, and preflight aerial lift checks. Assess work area procedures including testing, hot stick use, pole climbing, protective grounding, ladder safety, energy control lockout tagout, cover up, and confined spaces. Confirm PPE such as hard hat, safety glasses, FR clothing, rubber gloves test dates, fall protection, safety vest, face and eye and hearing protection, first aid kit, AED, fire extinguishers, and seat belts. Include tool condition, tailgate talks, housekeeping, storage, and comment sections with manager reviews and sign offs.
