Use this crew observation form to audit utility field crews and document safety and work practices. Record job type, date, preparer, location and list crew members. Verify compliance around energized lines, minimum approach distances, vehicle grounding, PPE in use, hotline tags and cover-up, a qualified observer, lockout/tagout, proper grounds, and completed job briefings and hazard reviews.
