This electric line crew observation template helps utility supervisors and safety leaders assess field practices for distribution work. Review critical controls such as PPE use, visible line openings, voltage testing, grounding, truck and outrigger grounding, non-reclose settings, and tag-out procedures. Check traffic control measures including signs, cones, vests, and parking setups. Confirm compliance with FR clothing, rubber gloves and sleeves, hard hats, hearing protection, wheel chocks, and harness use. Record job location, crew members, work order details, and tailgate status, add media evidence, and capture supervisor and foreman signatures to complete the observation.
