Power Quality Report Checklist

This power quality report template helps electrical utilities and field technicians document service and power quality issues. Capture inspection date and time, employee details, and member information including address, meter number, and reported issues such as blinking, dimming, flickering, bright or dim power, and outage frequency. Record service particulars like transformer KVA, service and entrance wire sizes and types, and open wire secondary details. Assess meter pan conditions and under-load voltages (left and right lug to ground, and phase-to-phase), noting connection status (OK, loose, corroded, rusted, burnt, replaced). Inspect weatherhead connections and service wire to the transformer. Measure transformer voltages (phase-to-ground and phase-to-phase) and review low- and high-side connections, hotline clamp condition, saddles, guards, fuse kit conversions, and bushing condition. Conclude with final assessment, comments, photos, sketches, and employee signature.
