Use this LUCA excavation checklist to audit trench and excavation safety on site. Verify utility locates and UPC dig tickets, require hand digging near marked lines, and ensure a competent person inspects conditions daily and after rain. Confirm proper sloping or trench boxes with tabulated data, safe ladder access within 25 feet, and spoil piles 2 feet from edges. Check barriers for unattended trenches, high-visibility apparel, safe distances from suspended loads, functioning equipment alarms, and required PPE.
