This Excavation Permit template helps teams plan and control trenching and excavation work. It captures permit details, site location, and the competent person in charge, then guides a pre-entry inspection covering utility locates, soil classification (default Class C per OSHA 1926.650–652 unless verified), need for shoring, sloping, or shielding, barricading, spoil placement (at least 2 feet from the edge), access and egress (ladders within 25 feet), water removal and monitoring, overhead hazards, and special precautions. The checklist reinforces daily inspections, stability checks for trench walls and slopes, correct bracing, and closure or secure barricading when not in use. Work may only begin after all NO findings are corrected and the competent person authorizes the permit.
