Use this A15 permit checklist to control work performed aloft or outboard on a vessel. Capture job details, location on board, and permit duration (max 24 hours). Verify bridge and comms isolations, weather limits under 30 knots, notifications, PPE and fall protection, tool control, and two points of attachment. Establish communications, assign authorising officer and competent person, and collect crew acceptance. Track issue and close-out, reactivation of equipment, removal of notices, and signatures.
