This walk through capture report template helps telecom field teams document assets and issues during network build surveys. Record PN area or feeder route, pole or chamber IDs, addresses or USRN references, and detailed issue descriptions such as bridge crossings, tree cutting, or D-poles. Capture level of impact, required actions, CCN and LOC details, redesign or access constraints, and any special engineering notes. Include Nokia comments as needed, attach photographic field evidence, and add map references to support remediation and handover.
