This TI A55 Report template helps field engineers survey telecom ducts and poles for network build or maintenance. Capture chamber types and locations, blockage points and distances, surface types, and required overlay meterage. Attach photos of chambers, ducts with rods, poles and IDs, plus client/Openreach map extracts and annotated overviews. Record whether overhead solutions are available, traffic management needs, speed limits, and any site-specific hazards. Document lead-in chambers, pole bends, DP IDs, duct section numbers, and proposed cable size in millimetres. Complete engineer sign-off to submit the A55 report.
