TI A55 Telecom Survey Report Template

This TI A55 Report template helps field engineers survey telecom ducts and poles for network build or maintenance. Capture chamber types and locations, blockage points and distances, surface types, and required overlay meterage. Attach photos of chambers, ducts with rods, poles and IDs, plus client/Openreach map extracts and annotated overviews. Record whether overhead solutions are available, traffic management needs, speed limits, and any site-specific hazards. Document lead-in chambers, pole bends, DP IDs, duct section numbers, and proposed cable size in millimetres. Complete engineer sign-off to submit the A55 report.

This Swish Yorkshire A55 template standardizes telecom network adjustment requests and surveys. Capture PIA NOI number, PN and asset affected, site contact and local authority. Record A55 purpose, overhead issues, poling help desk contact, alternative routes, and equipment to be added with dimensions. Log DP numbers, existing pole size and class, current and forecast wires, side load and private property status. Document chamber locations, surface types, traffic management needs, bores, use of 25mm rods, bore capacity, proposed cable or subduct, and duct type. Map locations using Eastings and Northings and what3words, with required photos of chambers, ducts and blockage points.
