Fiber Optic Splice Location Worksheet

This worksheet helps telecom crews document fiber optic splice locations and case details. Capture the job number, customer, and technicians on site; record the physical location, location type, case type and build, and case model references such as FOSC 450/600, Coyote, or 3M.
