This FTTx quality audit template helps field teams assess Fiber Access Terminal installations and maintenance. Capture the engineer name, CMP and FSA details, date, and site location. Inspect the FAT box condition, verify cable ties and sealant, check for feeder sag, and confirm pigtails are secured. Add photos of the open FAT, cable ties, feeder, and pigtails, and note any alarms. Document findings and escalate issues to MSPM, JCM, HC lead, CMM, or the FTTx lead as required to maintain network standards.
