This FTTx Fiber Access Terminal (FAT) quality audit template helps teams assess fiber drop installations and terminal boxes on site. Inspect FAT box condition, proper closure, and evidence with latitude and longitude. Verify cable management including use of cable ties, routing of drop cable loops inside the FAT, and absence of sags.
