Use this HTS Approved Variation (AV) Request form to submit claims for goodwill AV payments when unforeseen circumstances impact telecom field work. The checklist guides subcontractors through eligibility, including task types, validation with Tech Support or HSEQ, and required supporting evidence such as photos and notes. Non‑standard jobs, complaint liabilities, and rework of your own tasks are excluded. Claims must be submitted within two business days of job completion. Reasons covered include difficult cable runs, drilling internal walls, weather, tower outages, site access delays, and more, with reminders to contact Tech Support, use the travel calculator when applicable, and follow HSEQ procedures.
