Customer Connections Post-Installation Report

Standardized template for documenting customer connections after fiber installation. Capture site setup and pole setup, confirm job completion, and record MST and port numbers. Log light readings with required photos. For overhead jobs, attach photos of cable span, bracket or eyebolt, external cabling, wall-box terminations, and cable coils. For underground works, include chamber views, pre and post reinstatement, duct details, external wall-box, terminations, and cable coils. Add reasons for failure when applicable and any additional notes or images to support the report.
