Application for Payment (AFP) Evidence Checklist
This AFP evidence capture checklist supports contractor payment on completed telecommunications works. It guides field teams to record compliant photo evidence across cabinets, chambers, ducting and cabling, PIA activities, DNO attachments, MDU installs, active cabinets, Gigaclear poles, and reinstatement. Required images include site setup, marked blockages, damage and repairs, compliant reinstatement, and site clear conditions. Supervisors provide detailed summaries and outcomes, and each section includes specification checks and sign-off confirmations aligned with Gigaclear and Openreach standards, promoting traceability, quality, and safety.

Record report type, feature ID, exchange and location data, assess road speed and required traffic management, and confirm health and safety controls such as safe digs, hot job status, HV lines and confined space access.
