Use this Delap report template to document the condition of retail store equipment and fixtures. Record store and work order details, equipment type, make, model, serial number, age, and location. Capture cabinet dimensions and note electrics, mechanics, cosmetics, cleanliness, and overall condition. Add a narrative with repair or replacement recommendations. If replacing, document access issues, door apertures, time or parking restrictions, any racking or facade removal, socket type or hard‑wired connections, distance to power, and the locations of mains breakers and water shutoff. Attach photos of the asset (front and sides) and site access including the front of store.
