This commissioning sign-off checklist is designed for Boyles betting shops to verify a complete installation before handover. It covers dish installation and shop plans, dish locations and mounts, photos of dishes and cable routes, and detailed shop screen setup including gantries, SSBT and FOBT areas, numbers, EP/Boyles, football, CST, and window display screens with models, sizes, counts, and photos. Head-end checks include RF rack front and rear, 4G antenna location and power, WiFi access point, EPOS rack, BT line, and network connectivity. Counter and back-office checks capture till points, FOBT back office, printer location, phone installation and test, and credit card setup. The checklist concludes with client and engineer details and signatures for formal sign off.
