This municipal bylaws and compliance request for service form helps residents and officers record and track incidents that require follow up. Capture incident numbers, types such as signage, animals, environmental, TEPP, or general, along with incident details, priority, responsible officer, and dates. Log site information including location, owner name and address, and investigation findings. Document how and when the customer was contacted, officer decisions and actions taken, attach photos, and schedule follow up visits. Close the incident when resolved and record any follow up actions.
