Ce modèle Take 5 aide les équipes à marquer un temps d'arrêt avant l'intervention, clarifier la tâche, vérifier la formation, l'état des outils et la documentation requise, informer les personnes concernées et confirmer l'EPI adéquat. Il guide l'identification des dangers par type (atmosphérique, chimique, électricité, environnement, gravitationnel, manutention manuelle, éclairage, bruit, pression, radiation, thermique, autre), la capture de preuves photo, la description du risque, puis la définition d'actions correctives avec priorité et échéance à l'aide d'une matrice de risque.
