This STOP observation checklist based on the DuPont program helps teams record safe and unsafe acts, PPE use, equipment and tool condition, work area orderliness, and adherence to procedures. Capture general information such as site, area, shift, time spent, and people contacted or observed. Assess conditions across structures, environment, and orderliness; verify standards are available and adequate; and note reactions such as adjusting PPE or stopping work. Review positions of people, potential contact with hazards, and repetitive or awkward motions. Confirm tools are right for the job, used correctly, and in safe condition, and that procedures are known, understood, and followed to strengthen safety culture.
