Use this behavior-based safety observation checklist to capture at-risk and safe behaviors during routine work. Record the task observed, initial actions, eyes on task, speed of work, line of fire and body position, slips, trips and falls, struck-by and pinch points, PPE compliance, guards and rails, energy isolation (LOTO), and ergonomic body mechanics like lifting, bending, and repetitive motions. Verify procedures and standards are understood and followed, assess housekeeping, and confirm tools and equipment are correct, in safe condition, and used properly.
