This NCCU inpatient tracer helps teams prepare for Comprehensive Stroke Center certification. It guides audits of ED to NCCU handoff, documentation of last known well, symptom onset, and NIHSS on arrival. Verify imaging and laboratory turnaround times, tPA decision making and post therapy monitoring, swallow screening before oral intake, antithrombotic and lipid therapy at discharge, and VTE prophylaxis. It also covers SAH and ICH scoring, GCS, reversal protocols when indicated, consent and documentation for neuro procedures, and multidisciplinary care planning and patient education.
