MDU Survey Checklist
This MDU survey template supports planning fiber and broadband deployments in multi-dwelling buildings. Capture essential site information including site name, address, conducted date, and preparer. Record on-site contact and access requirements, take a picture of the building, and note the method of feed and presence of external chambers. Assess existing ducts into the property and internal duct locations, confirm if the MDU is roped, and identify any required civils activities. Document network-specific assets such as CityFibre toby boxes, Openreach poles, and distribution point (DP) locations with routes. Include THP count and detailed floorplans for lower ground through sixth floors to ensure accurate routing and installation planning.

Record project identifiers and site details, including PON reference, building name and address, and onsite contacts. Cover external works such as overhead spans, pole testing (DP/CP), duct requirements (54 mm and 96 mm across soft, footway, and carriageway), chambers, and traffic management. Document Openreach lead-in checks (test rodding, roping, blockages, A55), and any additional network sections tested.
