Use this incident audit to document damage to water infrastructure and related utilities during site works. Capture event details, location, photos, and root causes across procedure, equipment, method, human factors, and work conditions. Record notifications to the consultant, times of notification and discovery, and responsible personnel. Log water supply interruptions, shutdowns and take into operation, including MESKI notifications and staff. Track damaged utilities such as electricity, telecommunication, natural gas, irrigation, drainage and wastewater. Document repairs, materials used, costs, signatures from utility representatives, contractor and consultant, and engineer comments with recommendations to prevent recurrence.
