Use this street and pavement site inspection checklist to evaluate road surfaces, cracking and potholes, existing road shape, kerb and gutter condition, and the extent of linemarking replacement using thermoplastic or waterborne paint. Plan traffic control including stop go, detours, contra flow, road closures, and escort vehicles. Record low and high utilities, site hazards such as overhanging branches, low powerlines, gradient, lighting, parked cars, and access. Estimate profiler overburden removal and crew numbers, add photos and notes, and assess resident impacts across multiple sites.

Road Rehabilitation Inspection Report 2018/19
This road rehabilitation inspection report template helps civil works and local government teams assess road condition and plan maintenance. Capture road details including start and finish locations, road and block IDs, and block descriptions, then measure length, width, variable widths, and total area. Record crossfall, rideability, and overall ratings. Document pavement and surface failures—cracking, potholes, rutting, delamination, depressions, and service trench issues—with photos and mapped locations. This pavement site validation checklist supports onsite inspection of periodic rehabilitation projects. Record site details including location, GPS, SRRS and KM start–end, the existing surface and aggregate size, extent of cracking and patching, nearby assets and structures, and intersecting roads.
