This road rehabilitation inspection report template helps civil works and local government teams assess road condition and plan maintenance. Capture road details including start and finish locations, road and block IDs, and block descriptions, then measure length, width, variable widths, and total area. Record crossfall, rideability, and overall ratings. Document pavement and surface failures—cracking, potholes, rutting, delamination, depressions, and service trench issues—with photos and mapped locations. Specify required drainage works, kerb and gutter replacements, traffic devices, line marking, and ancillary tasks such as shoulder work, signage, and service lid adjustments. Select appropriate treatments including asphalt overlay, chip seal, microsurfacing, mill and fill, or rejuvenation to guide rehab planning.
