Use this inspection template to assess rolling ladders and portable scaffolding before use. Record equipment ID, material, size, department, location, inspection date, and preparer. Check steps, vertical bracing, platforms, casters, handrails and guardrails for damage, rust, corrosion, or missing parts. Verify labels, outriggers, and general condition. Apply the correct color zip tie when equipment is serviceable. If unsafe, attach a Do Not Operate tag, remove from service, and hold in a secure area until disposal or repair. Document findings, corrective actions, and sign off to maintain a safe, compliant workplace.
