Use this services completion sign off template to capture key details for in store works. Record the store location and client, confirm the signing in book and Health and Safety information have been completed and understood, and list all engineers on site with names and qualifications such as CSCS. Add pre works photos, confirm completion against the scope of works, note any additional works, on site or safety issues, and whether the store was handed back fit to trade. For each task, attach photos and details. Upload RAMS or safe system declarations, any required permits, and collect signatures from the lead engineer, additional personnel, and the store manager or main contractor, with dates, times, and sign off location.
