Monthly Ladder Check Template
Use this monthly ladder check template to assess the condition of ladders and stepladders before use. Verify there are no missing or loose steps or rungs, loose nails, screws or bolts, or cracked and twisted stiles. Inspect feet and hardware for wear or corrosion, document findings, add signatures, and track corrective actions. Ideal for construction, facilities, and maintenance teams to standardize inspections and improve compliance with working at heights safety.

This ladder monthly checklist helps teams perform consistent inspections of step, extension, platform, and fixed ladders. Record ladder ID and photos, then check for loose or damaged rungs and rails, slippery contaminants, wobble, loose hinges, missing or faulty dogs or pawls (extension locks), and signs of rust or corrosion. Capture findings, note defects, and assign corrective actions, then complete with date, name, and signature. Ideal for construction sites, facilities maintenance, warehouses, and utilities to standardize inspections, document evidence, and support safety compliance requirements.

Monthly Ladder Inspection Checklist
Ensure ladders are safe and compliant with this monthly ladder inspection checklist. Record identity tag and ladder ID, then check for loose or protruding rungs, nails, screws, bolts, or rivets; cracks or splits in uprights, braces, steps, or rungs; slivers; damaged or missing non-slip feet; and contamination such as oil, mud, paint, or corrosion. Inspect stepladders for wobble, bent or loose hinge spreaders, and broken stops. For telescopic ladders, verify latches operate correctly and frames are free from rust or corrosion. For work platforms, check hinges, stability, stops, and that the platform is not bent or cracked. If any defect is found, remove the ladder from service and label it until assessed by a competent person. Capture who carried out the inspection and the date to maintain monthly records.
