This ladder inspection form helps workers and safety teams evaluate step and extension ladders before use. Record the manufacturer, load rating, ladder type, and construction. Inspect for loose or damaged rungs and rails, missing or loose hardware, cracks, splinters, worn nonslip bases, and slippery contaminants. Confirm corrugation on metal rungs and sound wooden parts. For stepladders, check wobble, hinge spreaders, stops, and sharp edges; for extension ladders, verify lock operation, seat, and rope condition. Validate length limits by ladder type.
