This ladder inspection form helps crews assess the condition of ladders before use and document compliance. Verify manufacturer labels and IDs, check steps, rungs, rails, welds, and non-slip feet, and assess stepladder hinges and spreaders. For extension ladders, review locks, pulleys, ropes, and leveling devices; confirm platforms, rail supports, wheels, and cages are intact.
