This Field Level Hazard Assessment template helps crews identify job tasks, recognize hazards, assess risk, and define controls before work starts. It covers physical work area issues such as housekeeping, ground conditions, blocked exits, open excavation, confined spaces, hot work, vehicle and pedestrian traffic, and working at heights with fall arrest planning. Sections address electrical hazards, tool and equipment defects, ladders and scaffolds, lifting and critical lifts, ergonomic strain, chemical and biological exposure, noise, weather, lighting, fatigue, and remote work. It prompts permits, lockout tagout, safe work procedures, communication, and emergency preparedness.
