Use this Field Level Hazard Assessment (FLHA) template to plan work safely before starting a job. Capture company and site details, date and time, location, and the tasks being performed. Identify PPE requirements and confirm inspections. Assess hazards in the physical work area, environment, electrical systems, hazardous substances, biological exposure, equipment and tools, ergonomics, and people factors. Record risk ratings, select controls such as elimination, barricades, procedures, permits, lockout tagout, and awareness, and document how each hazard will be controlled. Cover high-risk activities including confined spaces, hot work, ground conditions, vehicle traffic, and working at heights with fall protection checks. Complete end-of-shift checks and sign off with notes on incidents and cleanup.
